Built for the way chronic illness actually works.

Flare-ups, brain fog, appointments on top of appointments — a regular planner isn't built for that. Ours is. We developed these with focus groups of people managing a wide range of long-term conditions, so the spreads reflect how you actually live.

These planners are undated — pick one up mid-flare, pause for a week, come back without wasted pages.

Inside: symptom and pain tracking, medication logs, appointment prep, questions for your care team, and gentle goal-setting that bends around bad days.

    Chronic Illness Planner FAQs

No! bloom's Chronic Illness Planner were created to support anyone navigating long-term health challenges, regardless of the specific diagnosis. We did extensive focus groups working with community members managing a wide range of conditions to make it as flexible and inclusive as possible.
Our Chronic Illness Planners are undated, which we did intentionally. Chronic illness doesn't follow a calendar. Some weeks you'll plan every day, others you'll need to step away entirely. Undated pages mean no wasted spreads and no guilt about "falling behind." Just pick up where you left off, whenever you're ready.
Each planner includes monthly, weekly, and daily layouts alongside dedicated pages for symptom and pain tracking, medication logs, appointment notes, questions for your doctor, and reflection prompts, and more. You'll also find space for goal-setting and reflection that's built around your energy, not against it.
Our classic planners focus on productivity, schedules, and goal-setting for a typical week. The Chronic Illness Planners keep the parts that work - monthly/weekly/daily layouts, gratitude prompts - but add health-tracking spreads you won't find in our other lines. The pacing of the pages is designed to accommodate bad days without shaming you for them.
Absolutely! Many of our customers do. Some use their Chronic Illness Planner as a dedicated health journal and keep a separate planner for work and life. Others bring the health pages to appointments as a conversation starter with their care team. Whatever works for your brain and your energy is the right way to use it.
